PCMH_4.3.14The Patient-Centered Medical Home is a progressive interdisciplinary team approach in which physicians, nurse practitioners, physician assistants, medical support personnel, patients and their families work together to yield the most effective, efficient, accessible healthcare possible.

By combining preventative care and the evaluation and management of acute and chronic conditions, the collaboration ensures patient-centered care of the highest quality.

National Recognition

As further testament to our efforts, the Wright Center for Primary Care Mid Valley location has been identified as an “exemplar practice” by a program funded by the Robert Wood Johnson Foundation called The Primary Care Team: Learning from Effective Ambulatory Practices (LEAP).

The goal of the LEAP Project is to identify and share innovative workforce practices that make primary care more accessible and effective for patients. The Wright Center for Primary Care Mid Valley is one of only 30 practices across the nation selected for this distinction.
