About the Webinar What does it take to be a high-functioning primary care practice in the US today? To answer this question, The Robert Wood Johnson Foundation and the MacColl Center for Health Care Innovation selected 30 practices, of all shapes and sizes, to participate in LEAP (Learning from Effective Ambulatory Practices). The sites were all chosen because of their exemplary and innovative staffing models, and on the next WIHI (Thursday, October 10, 2-3pm ET) we’ll take a look at the success factors that the project has identified thus far.
Guests include LEAP Project Director and creator of the Chronic Care Model, Dr. Ed Wagner; Drs. Kirsten Meisinger and Thad Schilling, whose participating sites are already earning accolades; and IHI’s Dr. Trissa Torres, herself no stranger to primary care transformation.
