The Wright Center for Community Health is a member of Aledade’s physician-led Accountable Care Organization (ACO). Aledade, one of the nation’s largest networks of independent primary care providers, collaborates with physician offices and health centers to enhance patient outcomes and generate sustainable revenue through a shared-risk model involving select Medicare Advantage and commercial plans.

Working alongside more than 1,900 practices, federally qualified health centers, and community health centers in 45 states and the District of Columbia, Aledade engages in value-based care through over 200 contracts, collectively impacting the lives of more than 2.5 million patients nationwide.

Aledade also equips physician offices with resources to promote preventive care, including routine annual wellness visits, statin prescriptions, diabetic lab monitoring, and ensuring patients receive recommended cancer screenings.
