赖特社区卫生中心因优先考虑质量改进和患者体验的医疗模式而获得人力资源和社会事务部的表彰
美国卫生与公众服务部下属的卫生资源与服务管理局（HRSA）最近授予赖特社区健康中心以 "以病人为中心的医疗之家"（PCMH）称号。
该表彰每年颁发给采用 PCMH 医疗模式的医疗中心，该模式优先考虑致力于持续质量改进和以患者为中心的医疗方法。
随着这一认可或徽章的获得，赖特中心今年通过联邦机构的社区卫生质量认可计划共获得了三个 HRSA 徽章。
此前，赖特中心还获得了 2022 年 "推进健康信息技术以提高质量 "徽章和新设立的 "应对健康的社会风险因素 "徽章。
HRSA annually reviews health centers’ performance data and bestows badges on federal Health Center Program awardees and Look-Alikes that have made notable quality improvement achievements in the areas of access, quality, and use of health information technology. HRSA encourages the recipients of its badges to prominently display them on the health centers’ websites and elsewhere, as outward symbols of the centers’ leadership in those key areas.
赖特中心此前采用了 PCMH 医疗模式，其在宾夕法尼亚州东北部的几家初级保健诊所接受了国家质量保证委员会的正式评估，并获得了 PCMH 认证。
Under the PCMH model of care, a patient is engaged in a direct relationship with a chosen physician or another provider who serves in a leadership role and coordinates a cooperative team of health care professionals. The leader takes responsibility for the comprehensive integrated care provided to the patient, and advocates and arranges appropriate care with other qualified providers, specialists, and community resources as needed.
研究表明，PCMH 可以提高医疗质量，改善患者体验，同时降低医疗成本。
赖特社区卫生中心于 2019 年成为人力资源和社会事务部指定的联邦合格医疗中心，目前在拉克瓦纳县、卢泽恩县和韦恩县运营着一个初级保健实践网络。
赖特中心提供安全网式、全面的初级和预防保健服务，包括医疗、牙科、行为健康、戒毒和康复以及传染病服务，涵盖从儿科到老年病的整个生命周期。该中心特别重视医疗服务不足的人群，不会因患者无力支付费用而将其拒之门外。
